BaseBall Alberta Incident Report Form New Incident Report form that is available on-line to be filled out when you have an incident. Starting in 2010, all umpires will be asked to submit any incident reports by filling out a form on-line (as opposed to a Word document). The beauty of this is that once you hit submit at the bottom of the form, the report gets directed immediately to me, so no longer do you have to fill out a form, attach it to a Word document, and email it. click here! |
