BaseBall Alberta Incident Report Form
(use this only for incidents which are under the auspices of Baseball Alberta)
(use the downloadable form below for all others -
this includes Little League, Babe Ruth etc)

New Incident Report form that is available on-line to be filled out when you have an incident. Starting in 2010, all umpires doing BA games, will be asked to submit any incident reports by filling out a form on-line (as opposed to a Word document). The beauty of this is that once you hit submit at the bottom of the form, the report gets directed immediately to BA, so no longer do you have to fill out a form, attach it to a Word document, and email it.
